To switch off all plug ins I tried, but no success. I have no idea, sorry. Looks like a JavaScript issue the form stay hidden after selecting the drop down…
Based on the information provided, it’s more than likely you have a theme and/or plugin conflict occurring which we’re unable to support. The best advice I can offer is to set wp_debug to ‘TRUE’ on your wp-config.php file and if you didn’t already, try temporarily reverting back to the default WP theme and deactivating all plugins other than ours to see if you can isolate the cause of conflict.
