Cannot collect attendee information per ticket

We are unable to use the attendee information feature as described here https://theeventscalendar.com/knowledgebase/collecting-attendee-information/.

Although we have created a field set of information we would like to be collected during checkout, there is no additional form or fields shown during checkout. How do we get this to work?

Thank you for sending that information over. I attempted to visit the link provided but the connection timed out :-/

Can you please confirm the issue still occurs when reviewing our Testing for conflicts guide?

Reason I ask is because we don’t have any logged bugs of users reporting similar issues and I’m unable to replicate it, so I just like to confirm it’s not a theme and/or plugin conflict.
