Radio buttons and checkbox not working on RSVP form

Hello,

when I am adding the attendee information to the RSVP form I do not see the radio button nor the checkbox. I only see the option entries for the checkbox as ‘yes’ and ‘no’ but no box or button where the user can select between the different options.

Any ideas what I can do to further investigate the issue?
