I have problems with the form for collecting custom info from the attendees:

It is not lining up correctly. The checkboxes should be each on separate line and the multi-line text box is in the middle of the text.

Can you help, please.

You’ll need to determine how best to overcome this issue with your theme, but typically it’s solved by adding CSS rules with more specificity to make things look how you want them.
