RSVP function not working correctly

    Hi,
    I have set up an free event where I would like to collect attendee information.
    I’m quite sure I have configured it correctly, but when I went to test it I get the following message on the user front end:
    “Please fill in the RSVP confirmation name and email fields” (please see attached pic)
    I have also attached a screen capture of the admin backend for your reference.
    It should be noted that it was previously functioning correctly prior to installing Event Tickets Plus.
