Need W9 from Modern Tribe to purchase Calendar Pro

I work for a public school district and we will need to add Modern Tribe as a vendor in our system before purchasing The Events Calendar Pro. This requires that we get specific forms from you, including a W9 and Conflict of Interest questionnaire (which we will provide).
