I had the plugin previously and when the update for the calendar was performed I could no longer find my page. So I deleted the plugin and reinstalled with the license number.
When I went in to my site to do as you asked, there was an update to the community plugin. Once I updated it, the form appeared like magic. Thanks for your assistance!
