As far as your other issue goes, if you look at the source – before it’s modified – what the plugin is sending is perfect:

So, it’s being manipulated after the fact by some Javascript, basically. Going through the troubleshooting steps of deactivating plagins and reverting to a default WP theme like Twenty Twelve should reveal the culprit.
