Have you looked the eCommerce system you are using for a plugin or help from them on how to add a field? Are you using WP eCommerce? I can try to look for a resource to help out if so, but I am limited in providing coding for customizations. Let me know. I found this resource on it: http://docs.wpecommerce.org/checkout/ The area is found here: Admin > Settings > Store > Checkout Tab Do you see that?
