Additional fields checkbox not working

I’m having a hard time getting the additional fields ( checkbox and radio) to work in other browsers such as
Google chrome
Safari

NP, Thks. Is this an issue with the theme possibly?

It very well could be. Have you tried switching to the default WordPress theme? Or, try deactivating all other plugins.

Unfortunately I have done all the above. We actually would like to continue using our current theme. Is that something that can be fixed?

Actually the script errors don’t seem to be the problem. It’s more a matter of CSS styling. You’ll need to go through and style things however you want them to be styled. Your theme is overriding some of our default styling so you’ll need to fix that.
